have an opportunity to review and edit your answers. |

| When you have submitted your
completed questionnaire, your data will be saved in a file. Later, you can
return to the file online with the User Name, Password and File Name. You
can then
download an RTF file (opens in Word and WordPerfect) with the customized
Organization Minutes and Bylaws ready for editing and printing. Please
print the page or record the File Name from the last page of the
questionnaire. |

| If you do not answer every question,
that field will be left blank in the documents. However, certain fields
must be completed for the Certificate of Incorporation. They are marked
“Required Fields.” The system will not move forward unless they are
filled in. Fields for the organization minutes and bylaws may be left
blank for completion later. |

| Shortly after you have submitted your
questionnaire, one of our corporate service representatives will review
the documents, proceed to check name availability and file the
corporation. You will be contacted by telephone or e-mail if we have any
questions. You will receive
an e-mail when the corporation has been filed, usually in one to two
business days. The corporate kit order will be entered immediately
after successful formation of the corporation. We will send the official
filing receipt and certificate of incorporation in the corporate kit
within one business day after we receive the official receipt. |
